Questionnaire

The questionnaire, schedule, and individual report are all available via the FitzMe vitality portal. 

Online questionnaire

The first step of the PMO is completing the online questionnaire. The questionnaire was developed in collaboration with, among others, the Dutch Heart Foundation and TNO. The questions are derived from validated questionnaires and measurement instruments. All requested themes are included in the questionnaire:

  • Physical activity: exercise, sports, physical strain, physical complaints
  • Lifestyle: cardiovascular history, diet, sleep, smoking, alcohol, drug use
  • Stress, resilience, PSA: stress, mental workload, job satisfaction, engagement, fit with work, importance of work, work-life balance, unwanted behavior
  • Work capacity: Work Ability Index
  • Other factors which influence employability: development opportunities, finances, informal care
  • Extra module: night work / shift work

 

Reporting

The report is where employees can find the results of the questionnaire and the physical examination. Using a clear and simple “traffic light model,” the results of the various measurements are interpreted and compared against the norm. As a result, each report is unique and provides every employee with tailored advice.

 

 

For each theme, the employee finds not only a color coding but also a comprehensive explanation of what the theme entails and what the outcome means. Additionally, the employee can immediately get to work based on this outcome. This advice adapts dynamically based on the measurement result, ensuring that every employee receives the right advice. This advice consists of the offerings on the FitzMe portal and can be supplemented with advice and follow-up programs from the provider's own network.

 

When employees receive long-term support with follow-up measurements (PMOs or checks), a history also unfolds in their report. This allows the employee to easily see the results of their efforts over an extended period. If desired, the employee can also download, save, and optionally print the report.
